If you have lost, damaged, or burned your Grade 8, Grade 10, or Grade 12 school certificates, you can obtain a replacement or a statement of results by following this simple and established process.
Steps to Replace Your Certificate
- Step 1: Obtain a Statutory Declaration Form
Visit your nearest courthouse and request a Statutory Declaration form. - Step 2: Provide Your Reason
On the form, clearly state why you need a replacement certificate. Common reasons include:- My certificate was lost
- My certificate was stolen
- My certificate was damaged or burned
- Step 3: Get the Declaration Signed
Take the completed form to a Commissioner of Oaths who will verify, sign, and stamp the form, making it legally valid. - Step 4: Submit to the Measurement Services Unit (MSU)
Bring the signed form to the MSU office, located opposite Sir John Guise Stadium in Port Moresby. MSU staff will assist you in printing a replacement certificate or providing a statement of results.

Replacement services are available only in Port Moresby. If you live outside the city, you can ask a family member or trusted person in Port Moresby to assist by sending them your signed documents.
